Integrated Community Care System (ICCS)
Our Integrated Community Care System (ICCS) work brings together health, local authority and third sector partners to deliver more joined-up, community-based care. Building on existing models such as @Home and Safe@Home, this work focuses on reducing fragmentation and supporting people to access the right care, in the right place, at the right time.
On the 4th March 2026, the ICCS Summit brought together partners from across health, local government, the third sector and community organisations at Glamorgan Cricket Ground in Sophia Gardens to explore how we can work differently to support people and communities across Cardiff and the Vale of Glamorgan.
The summit provided an opportunity for system leaders, practitioners and partners to share ideas, showcase innovation and discuss the practical steps needed to build a more connected, community-focused system of care. Through collaboration and shared learning, we explored how services can work more closely together to support prevention, early intervention and person-centred care.
